Middle and lower (cervico-facial) face-lift
" A shapelier face "

Rather than a long text, we thought we would answer the most commonly-asked questions as concretely and objectively as possible.

Dr Frederic Sarfati answers your questions
Ex Head of Clinic, Hospitals of Paris
Fellow of the Endoscopic Plastic Surgery Center Baltimore, USA

I) Before treatment Back

1.      Why would I have a middle and lower face lift ?

« As wrinkles form, the nasal and mouth lines may deepen, jowls appear and the mouth may droop at the edges. The skin gets thinner and looser and the face generally tends to lose its shape. »

2.     How will you find out what I need ?
« Before treatment we discuss what would be suitable. We use both recent and older photos and remorph them so I can advise you on how we get a natural-looking result that fits with your face. »

3.     Will a middle and lower face lift make me look younger ? How long do the effects last ?

« Your face should look about 10-12 years younger and the results should last 8-10 years. This is one of the most ambitious and yet the most natural ways of making a face look younger. »

4.     How will you get natural-looking results ?

« I want to make it clear we do not change your face's basic shape, from, say a triangular shape to an oval. We can only work on it if it has visibly lost some shape. The treatment is to naturally move the muscles upwards, without undue tension and remove excess skin to give the face a more even shape. »

5.     Will I need a medical examination before treatment ?

« During your preparatory consultation with the anaesthetist or the surgeon, we will ask you for the results of a general medical examination, to spot any medical contraindications. And you should take no aspirin or anti inflammatory medicines for ten days prior to your visit. »

II ) During treatment : Back

6.     How long will I stay at the clinic ?

« You will be hospitalized for a period of 24 hours. »

7.     How long will the treatment last ?

« Between three to four hours, depending on how much the skin and muscle has to be lifted. »

8.     What kind of anaesthetic do you recommend ?

« We normally recommend a general anaesthetic for greater comfort. A local could also be used. »

9.     What will happen during the treatment ?

« We tighten the facial skin and remove some of the fat from the cheek and lower jaw area. We reposition the cheek and lower jaw muscles to redefine the jaw line and we could also complete this with a reshaping of the cheeks and the chin. An upper and or lower eyelid lift could add to the effect and if necessary, we can give the face volume by fat injections. This middle and lower face-lift can be in addition to an Endoscopic lift of the upper face, which would retighten the skin and muscles of the upper cheek and forehead. »

10.     What will the risks involve ?

« There will certainly be bruising in this type of face lift. We prescribe antibiotics two days before the treatment until the seventh day after, just to make sure there is no infection. You may feel a slight reduction in sensation in the cheeks and jaw, which will go away after a few months. Any risks linked to the anaesthetic used will be discussed during pre-consultation with the anaesthetist. »

11.     What risks of phlebitis (deep vein thrombosis) are there ?

« There is a very low risk of phlebitis for this type of treatment. It consists of a blood clot appearing in the deep veins of the leg, which can detach and move to the lungs, causing a lung embolism. We can effectively deal with this risk using anti-coagulants and support stockings during your whole stay. During the treatment, we also do leg massages. So far, we have had no cases of phlebitis at all. »

The scars are really so discreet as to be almost invisible.

III ) After treatment : Back

12.     How visible will the scars be ?

« They will start at the top part of each ear, hidden behind the cartilage flap in front of the ear canal. Then they run round the bottom of the ears then back up, hidden in the crease behind the ear, right up into the hair line, avoiding the clear patch of skin behind the ear. We also often have to make a 2cm scar two to three cm behind the chin under the jaw. »

13.     Is it painful ?

« It is not a painful type of treatment, rated at 2.5 on a scale of 0-10 but the swelling caused is uncomfortable for a few days. »

14.     How quickly will I see the results ?

« After about three weeks you will see about 75%; residual swelling will take three to four months to go down. »

15.    How often will I need check ups ?

« We need to watch over the way things develop, take out various stitches and supporting steristrips on the sixth day, as well as other supports on the 15th day. Follow up lasts for a six month period, but we are also available at any other time. »

16.     What do you do to protect the treatment zone, do I have to wear something ?

« We use steristrips to maintain and support the face's new shape and limit swelling. We also limit eyelid swelling with wet bandages on the first night. We support the face with a press-lift (an elastic net balaclava which holds the lower face in place) for 15 days, then 15 more nights.
We recommend sleeping with several pillows to keep the head up. »

17.      What care should I take afterwards ?

« We will prescribe ordinary pain killers, Arnica granules and antibiotics for a few days and you will have to apply cold serum compresses for three to four days. »

18.     How soon can I start work again ?

« You can start work after three or four days but you will probably want to restart your social life only after 10-15 days. »

19.     Should I avoid physical activity ?

« You can start your sports again after a month, except diving, which is two months. »

20.     Is this treatment reimbursable on my health insurance ?

« You will have to consult your local health authorities and your private health insurers to see if you can be supported financially following a serious accident, for example. »
