Abdominal and hip liposuction
" Flat abdomen, slim hips "

Rather than a long text, we thought we would answer the most commonly-asked questions as concretely and objectively as possible.

Dr Frederic Sarfati answers your questions
Ex Head of Clinic, Hospitals of Paris
Fellow of the Endoscopic Plastic Surgery Center Baltimore, USA

I) Before treatment Back

1.      Why would I have abdominal and hip liposuction ?

« This procedure gets rid of localised fat deposits. Some may have appeared during adolescence, such as on the abdomen, or have appeared with time, lack of exercise or pregnancy. »

2.     How do you find out what my needs are ?
« Before treatment, we discuss what would be suitable. We use both recent and older photos and remorph them so I can advise you on how we get a nice natural-looking result that fits with your figure. »

3.   Does abdominal and hip liposuction make my figure look younger ?

« The skin reacts well in this procedure and will in most cases easily fit the new shape; this treatment is one of the most effective in making your figure look more youthful. Your waist will be higher, you should lose 1-3 sizes and be able to wear low-waist clothes again. Losing thigh weight also lifts your bottom, making it rounder, neater and more athletic, especially in a bikini. »

4.     4) How do you get natural-looking results ?

« The most important thing is to look at the skin's capacity to adjust to the new shape. As you have to look good in a bikini as well as dressed, it may just be preferable to keep some fullness of shape and make sure the skin is tight and smooth, rather than reduce too much and leave the skin too loose. Smooth results are a primary aim and we are able to do this by using very fine instruments, which do require high levels of skill. We also might look at abdominal muscle sag, and see if they would benefit from Endoscopic repositioning, a scarless procedure where we reposition the muscles. This would mean even higher quality results. »

5.     Do I need a medical examination before treatment ?

« During your preparatory consultation with the anaesthetist or the surgeon, we ask you for the results of a general medical examination to spot any medical contraindication. You should take no aspirin or anti inflammatory medicines for ten days prior to your visit. »

II ) During treatment : Back

6.     How long do I stay at the clinic ?

« We would either use a light epidural or a general anaesthetic which would mean an overnight stay. »

7.     How long does the treatment last ?

« This treatment lasts one and a half to three hours, depending on the work to be done. »

8.     What kind of anaesthetic do you recommend ?

« A light epidural or a general anaesthetic is needed for this type of procedure. »

9.     What happens during the treatment ?

« Firstly we inject an anaesthetic vasoconstriction fluid into the treatment zone. Then comes a fine tube of 3 or 4mm through a minute opening in the skin. Then we literally vacuum the fat out with it, moving it back and forth across the entire area to be treated. We would use a 3mm diameter tube for the abdomen and a 4mm diameter for the deeper fat layers. Using these fine instruments takes longer but results are much better. »

10.     What are the risks ?

« The only real risk is of bruising, which is quite frequent (haematomas being very rare). By prescribing antibiotics from two days before to seven days after treatment we eliminate risk of infection. You may find a decrease in sensitivity for a few months, which would absolutely not extend to the genitals. Very rarely a lymph pocket beneath the lower abdomen skin may swell up later, requiring lancing. Rare cases of ripple effect can be dealt with by massage or "m6" treatment. Very rarely we might carry out a mini liposuction at 6 months to finish off the work. Any risks associated with the anaesthetic will be clarified during pre-treatment interviews. »

11.     What risks of phlebitis (deep vein thrombosis) are there ?

« There is a very low risk of phlebitis for this type of treatment. It consists of a blood clot appearing in the deep veins of the leg, which can detach and move to the lungs, causing a lung embolism. We can effectively deal with this risk using anti-coagulants and support stockings during your whole stay. During the treatment, we also do leg massages. So far, we have had no cases of phlebitis at all. »

III ) After treatment : Back

12.     What scars will be visible ?

« There will be discreet, minor scarring, less than 8mm long each and nearly invisible after six months. It is hidden in the pubic fold in the pubic hair, in the navel and also on the lower back. It is closed by invisible sutures which we leave in place. »

13.     Will it be painful ?

« This is moderately painful treatment, rated at 3.5 out of 10 by patients on the abdomen and 4.5 on the hips. This is easily treated with normal pain killers. »

14.   How quickly will I see the results ?

« You will see most of the results (70% reduction) immediately but residual swelling will only disappear after about two to four weeks, depending on the patient. Pressotherapy sessions will help with this. »

15.    How often will I have to have check ups ?

« You will have a check up after 10 days, then once a month for three months. Of course you can make an appointment at any other time too. »

16.    What do you do to protect the treatment zone ? will I have to wear bandages ?

« You will have to wear a fitted elastic panty for two weeks, night and day. »

17.      What care should I take afterwards ?

« You can shower immediately. The little plasters covering the scars will drop off after 10 days or so, you do not have to replace them. We prescribe a treatment of iron and folic acid for a month to prevent anaemia. »

18.     How soon can I start work again ?

« You can start sedentary work after two to three days, but you must make no physical efforts at all. »

19.    For how long should I avoid physical activity ?

« You can start gym after four weeks and more strenuous activities after six. »

20.    Is this treatment reimbursable on my health insurance ?

« You will have to consult your local health authorities and your private health insurers to see if you can be supported financially following a serious illness resulting in weight loss, for example. »
