Nose
surgery - rhinoplasty
" A more fitting nose "
|
|
|
 |

Rather than a long text,
we thought we would answer
the most commonly-asked
questions as concretely
and objectively as possible.
Dr Frederic Sarfati answers
your questions
Ex Head of Clinic, Hospitals
of Paris
Fellow of the Endoscopic
Plastic Surgery Center Baltimore,

|
I) Before treatment
Back
|
1. Why would I have nose surgery ? |
|
« Mostly, the goal is to reshape the nose to make it better
looking by:
- Smoothing out bumps
- Reducing its contours,
its width, the outside
of the nostrils, and shortening
the end
- Repositioning the cartilage
to correct a breathing
problem. »
|
2. How will you find out what my needs are ?
|
|
« Before treatment, we discuss what would be suitable. We use both recent
and older photos and remorph
them so I can advise you
on how we get a nice natural-looking
result that fits with your
face (ie forehead angle,
chin projection, nostril
shape, cheekbones). »
|
3.
Will nose surgery make me look younger ? How long
do the effects last ?
|
|
« By reshaping the nose to fit the face better, we can make women's faces
look younger and men's
faces look more pleasant.
»
|
4.
How
will you get natural-looking
results ? |
|
« We have a lot of experience with this type of treatment; a good nose job
is delicate and skilful
and will give you the
desired results only if
properly done. This is
why there are rules we
must abide by and aim
for a realistic and natural
result. We have to take
into account skin flexibility
and the nose tip's cartilage
condition when we change
the nose shape. »
|
5.
Will I need a medical examination before treatment ?
|
|
« During
your preparatory consultation
with the anaesthetist
or the surgeon, we will
ask you for the results
of a general medical examination,
to spot any medical contraindications.
You should take no aspirin
or anti inflammatory medicines
for ten days prior to
your visit. »

II ) During treatment :
Back
|
6.
How
long will I stay at the
clinic ?
|
|
« You will have a 24 hour stay, needing
a post-treatment night
of observation. We remove
the humidity absorbing
nostril sponges the day
following the treatment.
»
|
7.
How long will the treatment last ?
|
|
« A nose
job lasts one to two hours,
depending on the work
that has to be done. »
|
8.
What kind of anaesthetic do you recommend ?
|
|
« We would generally do this under
general anaesthetic, except
when only working on the
nose tip, where we could
just use a local with
sedation. »
|
9.
What will happen during the treatment ? |
|
« We will be working on the size and shape of the nose, so this
means structural work
on bone and cartilage.
We will be reshaping the
profile by removing and
abrasing any irregularities
and reshaping the end
of the nose by working
on the nostril cartilage.
Where we are enlarging
the nose we transplant
cartilage from the septum,
the ear or more rarely
from the thorax. Sometimes
we use preformed prosthetics
for more radical augmentations. »
|
10.
What are the risks ?
|
|
« Bruising in the lower eyelids will appear,
which we successfully
treat using Arnica. Haematomas
, happily, are rare. We
use antibiotics to eliminate
any risk of infection
when transplanting cartilage
or inserting prosthetics.
Only about one in 50 patients
need retouching the profile
or nose tip, which would
occur about nine months
after the initial treatment.
»
|
11.
What risks of phlebitis (deep vein thrombosis)
are there ?
|
|
« There is a very low risk
of phlebitis for this
type of treatment. It
consists of a blood clot
appearing in the deep
veins of the leg, which
can detach and move to
the lungs, causing a lung
embolism. We can effectively
deal with this risk using
anti-coagulants and support
stockings during your
whole stay. During the
treatment, we also do
leg massages. So far,
we have had no cases of
phlebitis at all. »

III )
After treatment
:
Back

|
12.
Will there be any visible scars ?
|
|
« Any scarring will be inside the nostrils in the mucous
zone. If precision work
is needed on the nose
tip, there will be a discreet
scar between the nostrils.
If we are reducing the
nostrils, we place a short
scar on the lower edge
of the nose line, which
means it is practically
invisible. »
|
13.
Will it
be painful ?
|
|
« It
is uncomfortable, because
of the bruising and swelling
in the lower eyelids and
you may feel some difficulty
in breathing for a few
weeks. Any residual pain
is easily dealt with using
normal pain killers, the
nose job being rated at
about 2.5 on a scale of
0-10. »
|
|
|
|
|
|
|
14.
How
quickly will I see the results ?
|
|
« Results
are complete in two weeks,
but swelling will take
four months to completely
disappear. The nose will
further evolve over a
period of two years before
stabilising completely.
Full feeling in skin and
mucous membranes will
return after a few months
to a year. »
|
15. How
often will I have to come
back ?
|
|
« Your first check-up will be after about
a week when we will remove
plaster and any stitches.
We would then book another
in two weeks, then three
and nine months. We are
of course at your complete
disposition at any other
time. »
|
16.
What do you do to protect the treatment zone
? will I have to wear bandages ?
|
|
« We would
fit a type of plaster
cast to keep your nose
in place which you would
keep for a week. Sponges
in each nostril help the
mucous membranes to recover.
A facial press-lift is
not necessary. »
|
17.
What care should I take afterwards ?
|
|
« You can carry out most post treatment
care yourself at home.
Nose drops of evening
primrose oil will reduce
mucous inflammation and
help clear up scabbing.
We prescribe antibiotics
for five days. If you
still have difficulty
breathing after the third
week, vasocontricting
nose drops will help. »
|
18.
How
soon can I start work again ?
|
|
« You
can start work after three
days but the plaster cast
must stay on for an eight-day
period. After the first
week, most of the bruising
will have disappeared. »
|
19.
Should
I avoid physical activity ?
|
|
« You
can start home gymnastics
after the first week;
you should avoid activities
where your face might
get hit for a month. »
|
20. Is
this treatment reimbursable
on my health insurance ? |
|
« You
will have to consult your
local health authorities
and your private health
insurers to see if you
can be supported financially
following a serious accident,
for example. »
