Breast reduction and lift
" A natural-looking, reduced bust "

Rather than a long text, we thought we would answer the most commonly-asked questions as concretely and objectively as possible.

Dr Frederic Sarfati answers your questions
Ex Head of Clinic, Hospitals of Paris
Fellow of the Endoscopic Plastic Surgery Center Baltimore, USA

I) Before treatment Back

1.      Why would I have a breast reduction and lift ?

« If the size of your breasts is a problem for you. Heavy or asymmetrical breasts can come with age, pregnancy, weight loss or time. The advantages of surgical breast reduction are lowering bra-strap strain and shoulder, neck and back pain as the weight is reduced by the breasts' change in size and position. »

2.     How do you find out what my needs are ?
« Before treatment, we discuss what would be suitable. We use both recent and older photos and remorph them so I can advise you on how we get a nice natural-looking result that fits with your figure. »

3.    Does breast reduction make my figure look younger ?

« This treatment changes the size of your bust, so you have a more balanced figure. You will have more attractively shaped naturally younger looking breasts. You may find modifying this most feminine part of the body will have effects elsewhere, influencing your clothing habits, sporting and social life, also possibly your love life. »

4.     How do you get natural-looking results ?

« I carry out about 100 of these procedures a year and I make sure, through proper procedure and rigorous care and attention, that shape and symmetry is as natural as possible. »

5.     Do I need a medical examination before treatment ?

« During your preparatory consultation with the anaesthetist or the surgeon, we will ask you for the results of a general medical examination to spot any medical contraindications, including for example a mammogram for those over 38 years old or if there are particular problems in the breast region. You should take no aspirin or anti inflammatory medicines for ten days prior to your visit. »

II ) During treatment : Back

6.     How long would I stay at the clinic ?

« Hospitalisation is from 24 to 48 hours;
we only remove the drains the day after treatment.
»

7.     How long would the treatment last ?

« The treatment lasts about two to two and a half hours. »

8.     What kind of anaesthetic do you recommend ?

« A general anaesthetic is needed for this type of procedure. »

9.     What happens during the treatment ?

« We reduce, reshape and reposition the breast on the thorax and the skin is adapted to the new volume. We have to remove part of the breast tissue and we send this off for tests. The visible effect is of smaller, reshaped, higher breasts. »

10.     Will we see any scars ?

« There are two types of procedure, which lead to two different types of scarring. The first one, used in four out of ten cases leaves a scar round the areola and then vertically down to the breast/body crease. The vertical scar is slightly creased for a few weeks then is reabsorbed. »

Before and after a breast reduction with vertical scar.

After 16 months.

«  The other type of procedure is with an inverse T. It makes a scar around the areola, then vertically down to the breast/body crease, then a short (mostly around nine cm) horizontal line along the crease. This is used for when a large reduction is required or if the skin is very elastic and mobile »

Before and after a breast lift with T scar.

After 16 months.

11.     What will the risks be ?

« We take all active measures before, during and after treatment to reduce risks, namely of haematoma and infection, to the minimum. We prescribe antibiotics from two days before to seven days after treatment and carry out preventive surgical procedures during treatment. Sensitivity in the nipple and areola may be modified temporarily, or very rarely, permanently. This treatment does not at all encourage breast cancer. »

12.     What risks of phlebitis (deep vein thrombosis) would there be ?

« There is a very low risk of phlebitis for this type of treatment. It would consist of a blood clot appearing in the deep veins of the leg, which can detach and move to the lungs, causing a lung embolism. We can effectively deal with this risk using anti-coagulants and support stockings during your whole stay. During the treatment, we also do leg massages. So far, we have had no cases of phlebitis at all. »

III ) After treatment : Back

13.     Will it be painful ?

« This is not very painful treatment, patients measure it at 2.5 on a scale of 0-10. You will feel some discomfort due to the breasts' change of position and pulling. »

14.    How quickly will I see the results ?

« Results are immediately visible but the breasts will take two months to settle into place. Scars will be smoothed out in three weeks and after a vertical type procedure healing will be complete after two months. »

15.    How often would I have to come back ?

« You will have a check up after about 12 days, then one month, then every six months. Of course you can make an appointment at any other time too. »

16.   What will you do to protect the treatment zone ? will I have to wear bandages ?

« We apply waterproof bandaging which you keep for three weeks so you can take showers, but not hot baths. Then you would wear silicone bandaging, to improve the cosmetic healing, for a three week period. We recommend you wear seamless sports bras day and night for two weeks, then at night for two months. »

17.      What care should I take afterwards ?

« The only post-operational treatment you will need is antibiotics for five days and normal painkillers. We use deep seated invisible permanent stitching, and you will be able to shave off the visible ends yourself after a three week period. »

18.     How soon would I be able start work again ?

« You can start work after four days if you have a sedentary job, otherwise after two weeks. »

19.    Should I avoid physical activity ?

« You can start sports activities only after six weeks. »

20.    Will this treatment be reimbursable on my health insurance ?

« You will have to consult your local health authorities and your private health insurers to see if you can be supported financially following a serious illness or congenital deformation, for example. »
