The upper abdominoplasty or upper tummy tuck
" A flat, firm abdomen "

Rather than a long text, we thought we would answer the most commonly-asked questions as concretely and objectively as possible.

Dr Frederic Sarfati answers your questions
Ex Head of Clinic, Hospitals of Paris
Fellow of the Endoscopic Plastic Surgery Center Baltimore, USA

I) Before treatment Back

1.      Why would I have an upper tummy tuck ?

« This tuck is for any time your diet or exercise regime is not enough to get satisfactory results in the upper abdomen, or any time your body changes shape there. »

2.     How do you find out what my needs are ?
« Before treatment, we discuss what would be suitable. We use both recent and older photos and remorph them so I can advise you on how we get a nice natural-looking result that fits with your figure. »

3.     Does an upper tummy tuck really make my figure look younger ?

« Yes, this procedure gives you a slimmer, more youthful looking body. Recent progress in abdominal plastic surgery means that we can now deal with most aesthetic problems of the abdomen. The aim is for you to be able to wear low-waist clothes once again, showing your tummy, with no folds in the region above the belt, even when sitting. »

4.     How do you get natural-looking results ?

« The general aim is to recreate the lines and valleys that make an abdomen beautiful. Careful diagnosis tells us the best way to do this, depending on the state of the skin and of how the distribution of muscle and fat will work for us. The by now well-known procedure is to remove damaged skin and use only healthy, smooth skin to replace it. »

5.     Do I need a medical examination before treatment ?

« During your preparatory consultation with the anaesthetist or the surgeon, we will ask you for the results of a general medical examination to spot any medical contraindications. You should take no aspirin or anti inflammatory medicines for ten days prior to your visit. »

II ) During treatment : Back

6.     How long do I stay at the clinic ?

« Hospitalisation is for 24 hours as we need to leave the drains till the day after treatment. »

7.     How long does the treatment last ?

« The treatment lasts about two hours. »

8.     What kind of anaesthetic do you recommend ?

« A light epidural with neuroleptic-anaesthetic or else general anaesthetic is needed for this type of procedure. »

9.     What happens during the treatment ?

« We first of all take out the fat from the whole abdomen by liposuction. We will tighten the upper abdomen skin in one of two ways: if there less than about 8-9 cm of excess skin, then we tighten it upwards, fixing it using multiple deep sutures, the 5cm scar being in the breast/body fold. If there is more than 10cm of excess skin, then again we tighten it upwards, fixing it as before and this time taking out a band of skin in the breast/body fold. Then we reposition and smooth out the navel . »

10.     What are the risks ?

« We take all active measures to reduce risks to the minimum before, during and after the treatment, namely of haematoma (a one in 450 chance) and infection, by prescribing antibiotics from two days before to seven days after treatment as well as surgical measures during treatment. There may be risks due to diabetic obesity.
Sensitivity in the abdomen may be decreased, for a few months.
Rare cases of ripple effect can be dealt with by massage or so called m6 treatment. Any risks associated with the anaesthetic will be clarified during pre-treatment interviews. »

11.     What risks of phlebitis (deep vein thrombosis) are there ?

« There is a very low risk of phlebitis for this type of treatment. It consists of a blood clot appearing in the deep veins of the leg, which can detach and move to the lungs, causing a lung embolism. We can effectively deal with this risk using anti-coagulants and support stockings during your whole visit. During the treatment, we also do leg massages. So far, we have had no cases of phlebitis at all. »

III ) After treatment : Back

12.     What scars will be visible ?

« There will be a thin (1mm) discreet scar hidden in the breast/body fold. It will turn from pink to white over a period of 6-12 months. »

13.     Is it painful ?

« This procedure is measured at around 3 out of 10. Discomfort gradually disappears during the first week and you will be able to stop the painkillers as from the second week. »

14.     How soon will I be able to see the results ?

« You will see the results immediately in your figure's shape, but you may see the deep fixation points for a month. »

Results six months after an upper abdominal tuck.

15.    How often will I have to come back for check-ups ?

« You will need to be seen after two weeks, a month then two months. We are of course available to see you at any other time. »

16.     What do you do to protect the treatment zone ? will I have to wear bandages ?

« You will have to wear a special corset for two weeks. This is vital to support the reconstructed and repositioned muscles. »

17.      What care should I take afterwards ?

« There is not really any post operational care to carry out besides applying silicone adhesive strips to help with the cosmetic healing quality and taking antibiotics, anticoagulants, and normal pain-killers for two weeks. You can shower immediately. »

18.     How soon can I start work again ?

« You can start work after four days if you have a sedentary job, otherwise after 12 days. »

19.     Should I avoid physical activity ?

« You can start sports activities only after four to six weeks and ideally would start with gentle swimming. »

20.   Is this treatment reimbursable on my health insurance ?

« You will have to consult your local health authorities and your private health insurers to see if you can be supported financially following a serious illness or congenital deformation, for example. »
